Carry your passport the Vatican guided visit is nominative and name-checked at the desk, and both the Museums and St Peter's are airport-style security. Le Andra needs a scarf or shawl for shoulders.

St Peter's Square while it's still quiet. Stand on one of the two round porphyry discs in the pavement and Bernini's four rows of columns line up as one. Then a coffee in Prati.

At the Viale Vaticano entrance, 30 minutes before the 11:00. Show the voucher to staff at the reserved-booking line, right side of the main entrance. Through security, then the “Sportello Visite Guidate” (guided-visits desk) to collect the entry tickets.
Non-refundable, no changes. There are stairs on the route. Passports, water, the voucher on your phone.
Guided tour: Vatican Museums + Sistine Chapel. About three hours with a live guide and headsets. The Gallery of Maps, the Raphael Rooms, then the Sistine. Let the guide set the pace, the Museums are one-way and there is no doubling back.

St Peter's Basilica (if the tour didn't already take you in through the Sistine side door), free, ~45 min. The Pietà and John Paul II's tomb on the right. Then the dome climb, lift + 320 steps, pay at the entrance. Last admission is about 17:00 in October, so go up before you linger below.
Ponte and Castel Sant'Angelo at golden hour, Bernini's ten angels along the bridge, the papal fortress going floodlit. Interior + ramparts if the legs are willing (last entry ~18:30).

Julian Pizzarium is Gabriele Bonci's pizza-by-the-slice counter, the good thing near the Vatican that isn't Play-Doh. L'Arcangelo is the sit-down Roman option nearby.
Order: At Pizzarium: point at 3-4 squares (the potato-and-mozzarella, the mortadella-and-pistachio), a supplì; eat standing at the shelf.
Osteria delle Commari (Prati), booked, table for 2 at 20:00
house pickbooked Quiet, family-run, and priced for locals, the opposite of the tourist belt around the Vatican walls. Small, about 50 covers, worth booking rather than walking in. Their TheFork listing blocks automated booking, and their own domain redirected somewhere unrelated when we checked, and still does, so it was booked by email instead (commariroma@gmail.com). Via Santamaura 23, 00192 Roma, a few minutes' walk north of St Peter's, phone +39 06 3972 9557. Confirmed by the restaurant on 18 Sep, in Italian: Erica replied “le confermo la prenotazione”, I confirm the reservation. Mon 19 Oct, 20:00, 2 people, under Avitia. No deposit and no card taken, so if the day runs long, ring them rather than just arriving late.
Order: The carbonara, reviewers here call it one of the best they've ever had, the tonnarelli cacio e pepe, and the coda alla vaccinara (oxtail) for John's protein pick. A Castelli Romani white by the glass, ask what they're pouring.
Vatican Museums + Sistine, official guided visit (×2)
inbox, search “Vatican Museums” Be at the Viale Vaticano entrance by 10:30, reserved line on the right, then the Sportello Visite Guidate desk. Non-refundable. Shoulders and knees covered, enforced. Bag over 40×35×15cm goes to the free cloakroom. No photos at all in the Sistine Chapel.

An 11:00 guided tour skips the entry line entirely and puts you in the Sistine in good light. Keep the headset in, stay with the group, and don't peel off to re-photograph a room, there is no going back through the Museums.

Avoid this
Every food stand, truck and “food court” within 300 m of the Vatican walls, “it tastes like Play-Doh, we threw it away.” Eat in Prati proper. Ignore anyone selling “skip-the-line” tickets on the street.

Look for it on the walk
In The Creation of Adam, the shape around God and the angels is an exact cross-section of the human brain, folds, brainstem, pituitary. Michelangelo dissected corpses as a teenager; most scholars think it's deliberate: the spark being given is thought.
The Sistine's Last Judgment (the altar wall) has a flayed skin held by St Bartholomew, the face on the skin is Michelangelo's self-portrait. And the figure of Minos with donkey ears and a snake is a papal master of ceremonies who complained the nudes were obscene.
In St Peter's, the bronze statue of St Peter has a foot worn smooth and toeless from 700 years of pilgrims touching it. You can't any more, but you'll see where.
The things you'd wish you knew
The Sistine ceiling took four years (1508-12). Michelangelo painted it standing, neck bent back, not lying down, and wrote a bitter poem about how his beard pointed at heaven, his brush dripped in his face, and “I am not in a good place, and I am no painter.”
Dr. Trace and Dr. Leti did their Vatican the same way, a mid-morning guided tour with headsets. Their rule: let the guide lead and don't double back, the Museums are a one-way river and fighting the current is how a three-hour visit becomes five. There are water fountains inside, bring the bottle.
